Why is US Health Care so Expensive?

The situation with US health care is kind of like the old borscht belt joke in which a resort guest is complaining to another about the hotel’s restaurant:  “The food is just awful. . . .and they serve such small portions!”  According to most rankings, the quality of our health care is mediocre at best, yet we also have the most expensive health care on the planet.  Why is that?
